  • BY REGISTERING FOR THIS WEBSITE AND USING ANY OF THE TOOLS OR ASSESSMENTS OFFERED THROUGH LIFEWISEAC.COM AND IT'S PARTNER WEBSITES, YOU ACCEPT THE FOLLOWING TERMS AND CONDITIONS.

    What is personal information?
Personal information includes your demographic information (e.g., name, email address, date of birth, gender, member identification number and/or mailing address and zip code) and your medical information, including medical history and information contained on claims and health assessments.

How is personal information collected by LifeWiseAC.com and its partner websites?
LifeWiseAC.com may collect your personal information. Below is a brief description of each data source and the type of information collected.

    Information provided by you
Entering the Website. LifeWise’s server will specifically recognize the domain name and address of all site users. Our website transfers a text file to your computer's hard drive (known as a "cookie") for record keeping purposes. The cookie assigns a unique, random number to your computer. It does not contain information which personally identifies you. We record data on the categories or sections of our website that you have visited along with the cookie information.

Registering for the Website. The first time you enter the LifeWise web site, you will be recognized as either (i) a valid LifeWise member (ii) as a visitor, and offered an opportunity to register for the web site. You will be asked to create a personal login identifier and password with your first use. If you are a member, LifeWise will send your name, email address, date of birth, gender, member identification number and/or mailing address and zip code to partner sites. An account will be created for you and will be accessible to you each time you login. Each time you enter the LifeWise site (or its partner sites), you will be asked to login with that identifier and password. Access to your information by another user is strictly limited in accordance with applicable state and federal law, as well as LifeWise’ Notice of Privacy Practices.

Information provided by your health plan
LifeWise will directly, or indirectly through its business associates, provide personal information to its partners for health care operations activities, payment activities, and as otherwise allowed under HIPAA. Business associates are entities or individuals that are not employed by LifeWise that perform covered services on its behalf. LifeWise must have contracts with its business associates that require them to maintain the confidentiality of your personal information. For example, you can see if your recent office visit claim has been paid or you can see a list of all of the prescriptions you have filled in the recent past. Such prescription information is provided by LifeWise’s pharmacy benefit provider.

Information provided by your employer or health & wellness vendor
Your employer may have contracted with a vendor to administer a health or wellness program, which may include, but is not limited to, a health risk assessment. Depending on the terms of use of that program, the results of such campaigns and/or assessments, including but not limited to body fat mass index (BMI) and other health and lifestyle related measurements, may be forwarded to a partner site for inclusion in your Personal Health Record (PHR) or other sections of this site.

If you no longer desire to use LifeWiseAC.com (or its partner sites), we will endeavor to provide a way to correct, update or remove the personal information provided to us. We are required to maintain collected information for several years. To request help or discontinue service, please contact LifeWise Customer Service at the number on the back of your Member ID Card.

To whom can LifeWiseAC.com or its partner sites disclose my personal information?
LifeWise may disclose your personal information to business associates or third parties for health care operation activities, payment activities or as otherwise allowed or required by law without obtaining specific authorization from you. We may disclose personal information to your health care provider for the provider’s treatment, payment or health care operation activities. We may also disclose your personal information to your employer or its business associate to administer its wellness program or for its health care operations or payment activities under HIPAA. With your consent, we may also disclose your personal information to third parties in connection with wellness programs you elect to participate in. For example, we may provide your personal information to a third party in order to permit that third party to deliver a reward you earned. For more information on other disclosures that are allowed or required by law, see the LifeWise Notice of Privacy Practices.

What are my rights regarding personal information?

BY USING ANY OF THE TOOLS OR ASSESSMENTS OFFERED THROUGH THE WORLDDOC WEBSITE, YOU ACCEPT THE FOLLOWING TERMS AND CONDITIONS.

Participating in Activities and Tools on the Website. Some of the tools, such as the health risk assessment, request you to provide us with certain information including your, and your family's, medical history and health status. You may choose not to provide us with certain information, but that may limit the usefulness of the website for you to manage your health. For example, the tool may request information about your diet and exercise habits in order to identify new health behaviors to track.

Participating in Surveys & Contests on the Website. From time-to-time our site requests information from users via surveys. Participation in these surveys is completely voluntary and the user therefore has a choice whether or not to disclose this information. Information requested may include contact information and demographic information (e.g., name, email address, date of birth, gender, member identification number and/or mailing address, zip code). Survey information will be used for purposes of improving your experiences within our site.

Correcting or deleting information you provide. You have the opportunity to change or delete information that YOU have previously furnished and saved by you into your Personal Health Record (PHR). Your PHR includes the information entered by you using the tools and assessments on the web site. To change or delete information that you personally entered on the site, go to the PHR section. If you have any problems deleting or changing your information, you can contact LifeWise Customer Service at the number on the back of your Member ID Card. A link to LifeWise will also be provided that will allow you to correct personal demographic information (name, email address, date of birth, gender, member identification number and/or mailing address and zip code) about you sent to WorldDoc by LifeWise. Such information must be corrected within LifeWise and then the changed information will be resent to WorldDoc.

How does LifeWise or its partners use my personal information?
LifeWise or its partners will use your personal information to identify your individual health characteristics and display it in various sections of the website. For example, if you complete a Health Risk Assessment and indicate that you have heart disease, an identifier will indicate that you have this condition. LifeWise or its partners will use this information along with the rest of your data to determine whether to make specific recommendations to you in order to improve your health or to reduce your risk of experiencing an adverse event.

LifeWise or its partners may display the identifier and recommendations in your PHR or you may receive personalized emails from us. You may also be identified by means of specific calculations related to certain conditions, such as heart disease to receive personalized recommendations such as certain lab tests, health provider follow-up, diet, medication and other recommended guideline based treatment options suggested for your particular condition. When registering, you have the ability to check a box that states, "Yes, please send me periodic email messages with personalized health information, news and tips."

If you decide, after receiving an email, that you would not wish to receive these messages, you may "Opt-Out" of the service by: changing your email options in the Preferences area and clicking “Modify" or by calling LifeWise Customer Service at the number on the back of your Member ID Card.

LifeWise or its partners also send the user occasional site and service announcement updates. Members are not able to "Opt-Out” of service announcements as these contain important information about the service. LifeWise or its partners communicate via email or phone with users to reply to requests or to announce any significant changes relating to their accounts.

LifeWise or its partners may also use your personal information to carry on our own business planning and health care operations activities. We need to do this so we can provide you with benefits and ensure you receive the highest-quality services.

How does LifeWise or its partners use my personal information collected on this website?
LifeWise may use your personal information for its health care operations activities and other purposes as described in its Notice of Privacy Practices.

WorldDoc editorial policy and disclaimer information
Updated as of May 1, 2009.

    Medical information
All material on the WorldDoc site is developed and/or reviewed by over 20 WorldDoc physicians and researchers and third party providers that specialize in such online medical information. All material is grounded in evidence-based scientific research and the collective experience of practicing Board-certified physicians. Physician Authors who develop the health content on the WorldDoc website are licensed as Medical Doctors (M.D.), Doctors of Osteopathy (D.O.), Podiatrists, and other specialties and Board-certified in their designated specialty. Physician authors are not employees of WorldDoc and maintain active practice affiliations in their specialty. Where appropriate, WorldDoc provides a list of the physician authors that developed the health content and their current affiliations on the WorldDoc website.

Physician authors have an obligation to conduct business within guidelines that prohibit actual or potential conflicts of interest. A conflict of interest occurs when a physician is in a position to influence a decision that may result in personal gain as a result of suggesting health content. WorldDoc's two level review process and final approval by the Quality Oversight Committee limits any potential conflicts for physician authors. Health content material is dated to indicate when the material was last reviewed by a qualified medical professional for accuracy. The health content review process is extensive and performed annually to verify if it is still accurate and appropriate. WorldDoc keeps track of all content that is reviewed, edited and/or updated on the site. The information contained within the WorldDoc website is neither intended nor implied to be a substitute for professional medical advice, nor is it intended to be used for medical diagnosis, treatment or for emergency health situations. The service is presented for the sole purpose of disseminating health information. It is not intended and must not be taken to be the provision or practice of medical, nursing or professional health care advice or services in any jurisdiction. Always seek the advice of your physician or other qualified health provider prior to starting any new treatment or with any questions you may have regarding a medical condition. Medical assessment tools are designed using a "best practices" model which uses the combination of one or more medical content specialists from the specific content area, and a generalist physician reviewer. Our Personal Evaluation System is based on validated tools for identifying associated possibilities in order to educate individuals and promote better health decisions. Our Health Helpers section of the website contains tools based on long-standing, validated principles for advising people about risks and suggesting actions that could help restore and maintain their health.

    Pharmaceutical information
The information about drugs contained in this database is general in nature and is intended for use as an educational aid. It does not cover all possible uses, actions, precautions, side effects, or interactions of these medicines, nor is the information intended as medical advice for individual problems or for determining the risks and benefits of any individual taking a particular drug. Seek the care of a licensed healthcare professional to receive individualized medical advice and treatment.

    Explicit content warning
WorldDoc is intended for use by mature adults seeking understandable and accurate health information. When appropriate, photos and videos are used to enhance the educational value to WorldDoc users. All videos and photos (excluding diagrams and schematics) require the user to click an icon to activate the media. While most users will appreciate the value of seeing actual medical photographs and depictions, others may find the content offensive. WorldDoc and its physician authors have used discretion in choosing helpful visual depictions. If you are strongly opposed to content that may include exposed private parts of the anatomy and actual surgical procedures we advise you to not click any of the following icons on the WorldDoc website: video symbol

    Children's privacy
The Site is not intended or designed to attract children under the age of 13. We do not collect personal information from any person we actually know is a child under the age of 13. As set forth above, a parent or legal guardian may provide personal information about a child under the age of 13.

The material on this web site is provided for educational purposes and is not intended to replace the medical advice of your doctor or other healthcare providers. Only medical professionals who examine you can give you medical advice or diagnose your medical problem. We do not intend to create a physician-patient relationship. Your reliance on the information you get from WorldDoc is solely at your own risk. If you have a specific health question, contact your physician. Also note that while WorldDoc frequently updates its content, medical information changes rapidly. Therefore, some information may be out of date. If you have any concerns regarding our privacy policies or the source of our medical information, we welcome you to read our complete Privacy Notice.

    Source of funding/advertising
WorldDoc is funded through licensing agreements with entities such as large employers, health care organizations and individual users. Thus, unlike many other medical information websites, there is little or no advertising or third party promotion within most of the Website. Where WorldDoc has deemed it valuable to the end-user, we may offer links to entities that allow a user to purchase certain medically related items online. Any advertising or commerce-motivated content such as this will be presented to viewers in a manner and context that clearly differentiates promotional material from our medical information content.
